Please bring a referral letter, medicare card and ALL relevant pathology and imaging to your initial appointment.
If you have diabetes, please bring along your blood glucose readings and glucometer. Please bring along a recent list of your medications.
Please provide at least 24 HOURS NOTICE if your appointment needs to be re-scheduled.
There is a consultation fee if being seen in the private consulting rooms.
